Management in Healthcare conducts peer-review on how company’s post-acute analytics solution helped St. Joseph’s Health ACO achieve a 43% reduction in readmissions and $1.6M cost savings

BALTIMORE: Real Time Medical Systems (Real Time), the KLAS Rated, and HITRUST Certified post-acute analytics solution announced today its published article in the Management in Healthcare Journal, the major professional journal publishing in-depth, peer-reviewed articles and case studies on leadership, administration, and management in healthcare.

The article, “Improving patient outcomes while reducing readmissions with data analytics,” highlights how St. Joseph’s Health Accountable Care Organization (Mission Health Coordinated Care), implemented Real Time’s post-acute analytics platform and instituted a post-acute nurse navigator to manage their value-based patients in the post-acute setting. Results showed a 43% reduction in readmissions, 3.2 reduction in average length of stay, and a total cost of care savings of $1.6M as it relates to the Medicare population.
